Prize Draw - Terms and Conditions
1. The promoter is Sportswise (Goodform) Ltd and Premiership Rugby Ltd.
2. The promotion is a prize draw, which is open to anyone
who completes a Premiership Rugby club Fan Survey online.
3. No purchase necessary.
4. Entry is available through completion of a valid
survey only.
5. The 1st prize is an iPad.
6. No cash alternative will be offered.
7. Only one entry per household is permitted.
8. The closing date for entries is 12th
December. The winner will be the first entry drawn at random and will be
notified within a week of the survey closing date.
9. The draw is not open to employees of Premiership Rugby
Ltd, their subsidiaries, families, agents or their affiliates.
10. The prize winners may be expected to have their
photographs taken for press or promotion purposes.
11. Any queries and comments should be directed to Sportswise in writing to: Sportswise,

12. The promoter accepts no responsibility for entries
lost, damaged, delayed or unreadable and no such correspondence will be entered
into.
13. The name and county of the prize winners will be
available upon requesting in writing and sending a SAE to: Sportswise, 7 Olympus Court, Tachbrook,
Warwick, CV34 6RZ
14. Premiership Rugby Ltd reserves the right to
substitute the prize for one of its choosing which has a value the same or
greater than the original prize, in the event of circumstances outside of its
control. The prize is therefore subject to availability and change.
15. Entry into the draw will be deemed to constitute
acceptance of these terms and conditions.
16. Sportswise (Goodform) Ltd and Premiership Rugby Ltd will only retain
details of respondents who have opted-in to hear more from Premiership Rugby or
their club; however this will not negate or modify any consents previously
supplied by you to any individual club website (or other channel) to receive
communications from the club or Premiership Rugby. We will never share any of
this data with third parties.
